We, at the Palompon Institute of Technology, recognize our obligations under Rep. Act. No. 10173, otherwise known as the Data Privacy Act of 2012, with respect to data collection, data storage and data disposal from the student as well as new entrants. The personal data will be stored digitally in our School Automate System (SAS) portal while hard copies will be kept in a secure place in every office/unit.
Further, the information collected and stored in the portal shall only be used for the following purposes:
We shall retain data for a period of ten (10) years upon your graduation while a transcript of record will be kept in perpetuity. Personal Data will be disposed properly to avoid further processing.
The student has the right for access to, request for a copy of and update or rectifies his/her personal data. Corollary this, he/she has the right at any time to limit the processing and use of his/her personal data.
I have read and understood the Institute’s DATA PRIVACY STATEMENT and express my consent for the Palompon Institute of Technology to collect, record, read, organize, update or modify, retrieve, consolidate, block, erase or destruct my personal data.
I hereby affix my right to be informed, object to processing, access, and rectify, suspend or withdraw my personal data, and be indemnified in case of damages pursuant to provisions of R.A. No. 10173 of the Philippine or the Data Privacy Act of 2012 and its corresponding Implementing Rules and Regulations.
I, student, being fully aware of my duties and responsibilities as a student in the institution pledge to abide by the rules and regulations presently enforced as well as rules that may hereinafter be subsequently promulgated by the Department or by the Institute. To this end, I have voluntarily committed myself to uphold the integrity of the course/program that I enrolled by strictly observing the following:

The enrollment system will require you to upload the scanned images of the said documents above. Your enrollment application will only be processed once you submitted the hard copy of your Eligibility to Enroll document and other required documents.
Enclose and seal it in a long brown envelope and indicate your Reference ID, Name, and Program Applied (include the major if applicable).
